My health record

Melton Specialist Clinic uses a computer-based clinical support program that interacts with the Australian My Health Record.

Each eligible Australian will automatically have a My Health Record created. It is your choice to have a My Health Record and you can opt out at any time and have it permanently deleted.

Patients can elect to have a personally controlled and password protected health record. The My Health Record will allow health professionals in Australia to access important health information on a patient. Information included in a health record may consist of current medications, allergy information or any previous allergic reactions, an overview of your health, hospital discharge summaries, reports test & scans, referral letters, and demographic information including contact details, emergency contact details, indigenous status, and advanced care plan.

Your specialist is not required to obtain consent to view an individual’s record if the access is for the purpose of providing healthcare to the individual.
